Spring 2026 Admission Application Period: August 25 - September 29

Application steps:

Step 1: Online Application 

Online goes live on August 25, 2025 at 12 Midnight. Please fill out the online application form completely. Incomplete or late applications will not be considered. We are only accepting new applications. If you have applied previously, please apply again using this form.

Step 2: PDF Documents

Email all documents in one PDF file to email address: @email

A complete PDF file of your records should include:

1.  Cover page - 91������Ƭ�� Nursing Program Application Checklist.
   91������Ƭ�� Nursing Application Checklist_Spring 2026
2. A copy of your California Driver's License/Identification Card and 91������Ƭ��/91������Ƭ�� Student Identification Card (if available).
3. The 91������Ƭ�� Multi-Criteria Screening Process for Admission Worksheet. Please fill out completely and tally score on page 3.

4. ATI TEAS Test Version 7 Proctored Assessment Individual Scores > or = to 62.0%. in all four subcategories 
   • First score > or = to 62.0%. A higher repeat TEAS score will not be considered
   • All four TEAS sub-categories test score print-out must be submitted
5. High school and college transcripts.
   • Please yellow highlight courses taken that meet the admission requirements
   • Unofficial transcripts may be submitted.
   • Please do not submit official transcripts until you have been instructed to do so
6. Additional supporting documents per the 91������Ƭ�� Multi-Criteria Screening Process for Admission Worksheet.
7. Copy of COVID vaccine including booster shot (optional)
8. Nursing counseling worksheet - RN Program Course Information (optional)
